Post-Acute Performance Coach

Louisville, KY (In-Person) Full-time $65,000 – $85,000

The Role

You are the person clients trust. As a Post-Acute Performance Coach, you'll be the primary relationship with home health and hospice agencies — embedded in their operations, driving measurable improvements in their revenue cycle, and helping them build internal capabilities they didn't know were possible.

This isn't advisory work from a distance. You'll join weekly calls with client teams, lead process improvement initiatives, train staff on new workflows, and work side by side with our Automation Trainers to deploy the right technology at the right time. Your deep understanding of post-acute revenue cycle is the foundation — our platform and team amplify it.

What You'll Do

  • Lead weekly coaching engagements with client agencies — at least three structured meetings per week across clinical operations, financial operations, and CFO one-on-ones.
  • Deliver the deep analysis presentation and action item report to new clients, translating complex data findings into clear, prioritized next steps.
  • Drive process improvement, standardization, and system configuration changes across client internal staff and external vendors.
  • Join calls between clients and their external partners (billing companies, clearinghouses, payer representatives) as a trusted advisor.
  • Collaborate with Automation Trainers on the sequencing and rollout of automation across client systems.
  • Monitor performance improvement, track change management progress, and report on vendor reduction and cost savings.
  • Identify bottlenecks in client processes, root-cause system configuration issues, and surface payer-specific problems affecting revenue.

What You Bring

  • 5+ years in post-acute revenue cycle — home health and/or hospice required.
  • Deep working knowledge of the full revenue cycle: patient intake, authorization, billing, AR, denial management, payment posting, and documentation flow.
  • Experience with major EMR systems — WellSky, Homecare Homebase, Axxess, MatrixCare, PointClickCare, or similar.
  • Strong understanding of coding and chart review requirements in post-acute care.
  • Cross-functional experience — you've worked across clinical and financial operations, not just one side.
  • Ambition, a strong desire to lead, and genuine passion for making healthcare operations work better.

  • Professional certifications. They're nice to have, but they're not a gate.
